Dear Doctors

We have exciting news!!

We have just finished a NEW Posture Module with Biotonix. This New Postural Module (called the PosturePrint® ) is a computer analysis of posture in terms of rotations in degrees and translations in millimeters. The New Computer Code was written by Tad Janik, PhD, who has done all the CBP Spinal Modeling in the past.

This New Module requires 3 digital photos (left lateral, AP, right lateral), with 14 spherical markers placed on the patient, and 15 points to be determined via computer mouse by the doctor. Importantly, the patient’s abnormal postures of the head, rib cage, and pelvis are linked to the CBP Mirror Image® exercises.

This New Postural Module at Biotonix includes a Report of Findings, written by Dr. Deed, termed the PosturePrint® and a re-evaluation report, termed the Comparative Report. Since the procedure (posture photos, postural analysis, face-to-face Report of Findings with Doctor & patient) has a written Report, this enables three fundamental necessities:

  1. Patient has a written document explaining their postural problems
    • Explains long-term structural rehab care
    • Has personal photos & explanation of their condition
    • Can be shown to friends & family to stimulate referrals
    • Has personalized exercise program
    • Has a Medical Necessity section for Computerized measurements of posture
  1. Copy of this document can be sent to 3 rd party payers to substantiate rehab care
  2. This procedure is reimbursable from many 3 rd party payers

To illustrate the billing of this entire procedure, Deed found some E & M Codes that apply (usually, this Report must be given on a different day than the X-ray Report of Findings to justify a separate billing):

  • EM Counseling: 99211-25 (5 min) =  Report of Fndings = service doc-patient face to face counseling; (in Nevada, the relative value unit is RVU = 0.59) = [Discussion with patient about one of more of "Diagnostic Results, Impressions, Prognosis, Risks/Benefits of management, instructions for management, and or follow-up”]. The modifier -25 means that this code is a separate service, i.e. a Report of findings. So if the DC went through the new CBP Postural Report and then showed them the exercises, it would be an approximate 15 minute service.
  • Depending on the time used to explain their posture and Mirror Image® exercises, there are increasing RVU amounts possible:
  • 99212-25 (10 min) = "; (RVU = 1.03)
  • 99213-25 (15 min) = ", (RVU = 1.42)
  • 99214-25 (20 min)
  • 99215-25 (25 min).
  • Code 97750 Physical performance test or measurement complete with written Report (relative value unit = 0.77). For examples in Nevada, from low-medium-better, one unit is $56USA; for 99213, we have $56 x 1.42 = $79.52....and for 97750, we get $56 x 0.77 = $43.12.

Additionally, it appears to us that:

  • A DC could bill for both codes:
    • 99212 + 97750 = $57.68 + $43.12 = $100.80 OR
    • 99213 + 97750 = $79.52 + $43.12 = $122.64.
  • A DC could bill for both codes because 97750 is posture evaluation with written report and one of the 9921 -* is a face to face report of finding.

This important aspect of the New Biotonix Module means the DC might make $122.64 x (80% for some insurance) - $30 for the Biotonix website analysis = $98.11 - $30 = $68 profit on each postural evaluation with coverage by a 3 rd party payer.

Besides having a great analysis, besides having a Report that helps with patient retention, besides having a document that determines, illustrates, and explains the patient’s Mirror Image® exercises, the DC can make a profit whenever there is 3 rd party payer coverage.

Dr. Deed Harrison and Dr. Martin Normand (Univ Quebec) have written the exercise explanations to involve increasing exercise difficulty in 3 blocks of time in a 9-week period. Thus, there will be a need to Re-evaluate the patient’s posture at 9 weeks of Rehab Care. To make this another billable procedure, Dr. Deed has written a Comparative PosturePrint® Report that:

  1. Measures and compares Patient’s Posture Improvements in Tables;
  2. Inserts before and after AP & Lateral photos of the patients,
  3. Updates the patient’s exercises.

ICA and Biotonix Reach a Cooperative Agreement to Conduct A Large Posture Research Study

By Donald Hirsh, MS, DC, FICA

The ICA’s Posture Committee and Biotonix are planning the most significant study in chiropractic history on the relationship between health and postural displacements. The research project will be directed by me, Deed Harrison, DC, and Burt Holland, PhD. (Statistician at Temple University). Dr. Denise Perron (Biotonix) will be providing the state of the art digital posture analysis equipment for the study as well as significant infrastructure support.

This research, in the planning stage for two years, will involve at least 2,000 subjects. Each subject will have a digital postural scan using the latest PosturePrint® technology and they will fill out a comprehensive health, as well as pain, questionnaire. The premise of the study is that there is no correlation between poor posture and poor health and pain. As in all good science, the goal is to try to prove yourself wrong. The study is being carefully designed to produce the most robust conclusions. Half of the subjects will self-select chiropractic care as new patients to volunteer chiropractors selected using rigorous requirements by ICA’s Posture Committee. The other half of the subjects will be friends or relatives of the first group that are of similar age and the same gender, but they have not sought chiropractic care.

Biotonix has created a new postural module to their system, called the PosturePrint® in order to accurately collect the data needed for this study. They obtained the aid of Tad Janik, PhD (Numerical Analyst in Alabama) to perfect an accurate computer program to evaluate posture from three photographs (AP view and both lateral views). Biotonix has agreed to partner this posture project by making the data results accessible to the ICA Posture Committee and Dr. Holland for statistical analysis.

Currently, reliability and repeatability studies are ongoing to insure the validity of this research. Once these studies are completed, the ICA’s Posture Committee will publish a request for doctors of chiropractic to volunteer as data collectors. Rigorous standards for selection and for training will be applied. The volunteer doctors, besides the professional gratification, will receive complementary training in the state of the art posture analysis technology as well as significant discounts on the continued use of the PosturePrint® reports for their existing patient base. This new Report, written by successful practitioners, can help with patient education and it provides corrective exercises, individualized for each patient.
